Adenopathy must be differentiated from the thymus, retrosternal thyroid, developmental cysts and solid mediastinal tumors, including neurogenic tumors, germ cell tumors and thymomas. The differential diagnosis of enlarged lymph nodes must also include lymphoma, especially Hodgkin's disease in the older child. These nodes tend to be large, lobulated, homogeneous, do not typically peripherally enhance with intravenous contrast studies and involve predominantly the anterior mediastinum.
